- 1). Go to the D.C. Department of Health website to complete the online birth certificate request form. You cannot submit this form via the Internet, you must print it.
- 2). Gather necessary documentation to submit with the request form. The type of documents you need depend on your relationship to the person whose birth certificate you are requesting. You will only need a copy of your driver's license if you are ordering your own birth certificate or one for your child if he or she is less than 21 years of age. Another type of government-issued photo identification, such as a passport, is also acceptable. For a child who is 21 or older or for one of your siblings, you will need to send a notarized statement of the registrant granting you approval to obtain the birth certificate. If you are a grandparent or legal guardian, you will need to send documentation proving you are the registrant's legal guardian.
- 3). Mail the request form, payment, required documentation and a self-addressed stamped envelope to the D.C. Department of Health. As of November 2010, the fee for ordering a birth certificate is $23.
