How to Apply for a Passport for Ten Year Old Daughter

    • 1). Complete Form DS-11: Application For A U.S. Passport and submit the passport in person at an acceptance facility or passport agency. You must take your with you when you submit your application. The application should not be signed when you submit it as it must be signed in front of an acceptance facility or passport agent by your child. The child's social security number is required for the application. As of April 2011 the fee for a child passport book and card is $120, $105 for a passport book and $40 for a passport card. Fees must be paid when the application is submitted.

    • 2). Submit the evidence of U.S. Citizenship for the child. Evidence of citizenship includes a previously issued undamaged passport, a certified birth certificate issued by the city, state, or country of nationality, a Consular Report of Birth Abroad or Certification of Birth, a naturalization certificate or a certificate of citizenship.

    • 3). Submit proof of relationship to the child. Acceptable proof of relationship includes the child's birth certificate with both parents' name listed, a foreign certificate of birth with both parents' name on it, the child's report of birth abroad with both parents' name on it, an adoption decree with both parents' name on it, court order establishing custody or court order establishing guardianship.

    • 4). Submit acceptable forms of identification and photocopies of the form of identification submitted for both the child's parents. The photocopy must be on 8 ½ x 11 paper and show the front and back of the submitted identification with the copied images on one side of the paper. Acceptable forms of identification include a previously issued undamaged passport, naturalization certificate, valid driver's license, current government employee ID or current military ID.

    • 5). Submit proof of parental consent. If both parents are not in attendance when the child's application is submitted it then the applying parent must provide proof of the other parent's consent. If the parent does not have a court order showing sole consent they must provide a certified copy of the Statement of Consent that has been completed by the non-applicant parent. Parents can also submit any of the acceptable forms of identification with only the apply parent name on it as proof for consent purposes.

    • 6). Submit one 2 x 2 inch photograph of the applicant child. The photo must not be more than six months old, full-face viewing forward and in color with normal daily wear clothing on. Uniforms should not be worn in the photo unless they are religious in nature and worn daily. All of the listed documents must be submitted with the passport application in order for the application to be considered. All documents collected during the application process are returned by mail. Passports are delivered by mail in a separate package.
