- 1). Compile the necessary information needed to fill out your Adult Disability Report through the Social Security Administration. You will need the names and information of any doctor, hospital or pharmacy that has treated you for your disability. The SSA will need your patient ID number and all treatment dates that you have in your possession. Include any medicines you are currently taking, or have taken in the past, along with any medical tests.
- 2). Fill out the Adult Disability and Work History Report available online at the SSA website. To start the report, click "Start Report" located at the right of your screen. You will provide the information you gathered in Step 1, as well as the last five places in which you have worked.
- 3). Click "Submit" once you have reviewed the Adult Disability and Work History Report.
- 4). Fill out an "Authorization to Disclose Information to the Social Security Administration" document. The document PDF file is available on the SSA website. This document will allow the administration to process and approve your application so that you may receive your benefits.
- 5). Mail your "Authorization" form to your local Social Security office. You can also deliver the form to the office in person. Processing takes a few weeks, and it may be two to three months before you receive your benefits.
