- 1). Obtain Publication M-476, A Guide to Naturalization, from the United States Citizen and Immigration Services (USCIS) via their website (see Resource section) or by calling 800-870-3676. This booklet will help you determine your eligibility to apply for naturalization using Form N-400.
- 2). Complete Part One of N-400. Fill in your legal name (the name that is on your birth certificate, unless it has been legally changed through marriage or a legal name change petition) and your name as it appears on your current Permanent Resident card. If you have used any other names, include those in this section. If you wish to have your name legally changed after you are naturalized, indicate what you want your name to be. The name change will become official when your naturalization is approved.
- 3). Indicate why you think you are eligible for naturalization by checking the appropriate box in Part Two. If your reason is not listed, check "other" and detail your eligibility justification in the space provided.
- 4). Add personal information in Part Three. Provide your Social Security number, if you have one, date of birth, date you became a Lawful Permanent Resident, country of birth, country of nationality, parents' citizenship and your marital status. If you are disabled and need a waiver to excuse you from taking the naturalization tests, or if you will need special accommodations to take the test, indicate that in this section. All dates should be written as month/day/four-digit year.
- 5). Put your address and phone number in Part Four.
- 6). Check the boxes that best match descriptions of your appearance, race and gender in Part Five. This information alone does not impact a decision on your application, but is used by the FBI in conjunction with your fingerprints to conduct a criminal background check.
- 7). Supply information about every place you have lived and worked in during the five years leading up your application in Part Six.
- 8). Complete Part Seven by providing information about every trip you have taken out of the country that lasted longer than 24 hours in the past five years, and adding up the total days you have been outside the U.S. during that time.
- 9). List all of your marriages, divorces and annulments in Part Eight, along with information about the citizenship of your former and current spouse(s).

Provide a list of all your children, whether they are alive, deceased or missing, and all the information you can about their citizenship and where they live in Part Nine. - 11
Answer the questions about background and criminal history in Part Ten by checking the appropriate boxes. It is important to be truthful. If you are not, your application may be denied. - 12
Sign the form in Part Eleven. If someone helped you to complete the form, they should sign Part Twelve. Submit your form to:
