- All Pennsylvania residents who earn above a certain income level during the previous tax year must fill out and submit the PA-40 form to file their state income tax return. For 2009 income taxes, any state resident who earned more than $33 in gross taxable income must fill out and submit the form to the state Department of Revenue. Anyone who incurred a loss in a business transaction as an individual, sole proprietor, partner in a legal business partnership or an S corp shareholder. Minors and deceased persons who meet these requirements must also file Form PA-40; a representative should fill out and file the form on behalf of any deceased persons.
- When filing Form PA-40 with the Pennsylvania Department of Revenue, a taxpayer must often prepare additional forms to be submitted with his tax return. Any taxpayer claiming income tax credits on taxes paid to other states or countries must fill out and submit the appropriate additional schedule forms, which range from Schedule G to Schedule R. These forms must immediately follow form PA-40 in your submission. Income tax returns should also include your W-2 forms, PA Schedule OC for any additional Pennsylvania state tax credits as well as your tax payment voucher.
- The Pennsylvania Department of Revenue allows taxpayers to file Form PA-40 electronically in three different ways. Taxpayers who filed Form PA-40 in the past year can file their income tax return over the telephone; to file over the phone, a Pennsylvania resident must enter his Social Security number and ZIP code. Pennsylvania taxpayers can also fill out Form PA-40 online and submit it through the Pennsylvania Department of Revenue's official website. Form PA-40 is also compatible with the e-file software system that United States federal taxes and many other states utilize; compatible tax preparation software allows a taxpayer to file to the Department of Revenue through the software program.
- If completing a paper Form PA-40, a taxpayer must mail the form and all other appropriate documents to the proper receiving address depending on whether the taxpayer will be receiving a refund or will have to make an additional payment. All income tax returns should be mailed to Pennsylvania Department of Revenue offices in Harrisburg. Taxpayers receiving a refund, those who owe money on their taxes and taxpayers who don't owe or receive any money to the Department of Revenue each mail their tax returns to different addresses. All addresses are available on the Department of Revenue's website.
