How to Obtain the Military Record of Veterans

    • 1). Acquire the Request Pertaining to Military Records from (SF 180). To request an SF 180, write to the National Personnel Records Center, 9700 Page Ave, St. Louis, Missouri 63132 or download and print the form located on the National Archives website.

    • 2). Read the Instruction and Information Sheet for SF 180, Request Pertaining to Military Records located at the top of the sheet before filling out the form. This section includes additional instructions and general information concerning military records. Included is what information can be released and any fees involved to get that information.

    • 3). Fill out the SF 180 form with as much pertinent information available specific to the sections of the form. The form is self-explanatory but may appear crowded with sections and boxes. Find a quiet, uninterrupted time to accomplish the task.

    • 4). Review the form to make sure all appropriate boxes are checked and applicable sections are complete with the correct data. Missing information requested on the form can slow down the process.

    • 5). Sign and date the SF 180 and mail to the address located at the bottom right corner of the page. The veteran or next of kin's signature must be present on the form before the personnel records department can send any information requested. Be sure to include the return address of where to mail the requested information.
