    Meeting the Eligibility Requirements

    • 1). Determine your eligibility to receive a birth certificate by being the child named on the certificate and older than 18, being the parent, guardian or legal representative of the person named on the certificate, or having a court order.

    • 2). Supply information about the child on the certificate by providing the full name of child at birth or any legal name changes, the state, city or county of the child's birth, the mother's full name including a maiden surname, and the father's full name if on the record.

    • 3). Give information about the person ordering the certificate by providing that person's full name, the relationship to the person named on the certificate, the complete mailing address and phone number of where to contact the person, and the applicant's signature.

    • 4). Visit the state's website to make the online order when applicant meets all the eligibility requirements, and have information on hand to input in the online ordering forms.
