By Mail and In Person

• 1). Obtain an application for a Maryland marriage certificate from the Vital Statistics Administration website by downloading the PDF version or the Microsoft Word version of the application on your computer (vsa.maryland.gov). This application is for marriages occurring after 1990.
  
  
• 2). Complete the application by providing the name of the bride and groom, the date of the marriage, the county of the marriage and your personal information. Provide your phone number, mailing address and your relationship to the couple listed on the marriage record.
  
  
• 3). Make a copy of your photo identification. The Vital Statistics Administration accepts driver's licenses, state ID cards, passports and other forms of photo ID issued by the U.S. government. If you are unable to provide photo ID, you may provide two alternative forms of identification that state your name and current mailing address, such as a W2, paystub, utility bill, mortgage statement or vehicle registration.
  
  
• 4). Write a check or money order in the amount of your order. Each copy of a Maryland marriage record is $12 as of 2011. Make your check payable to the Division of Vital Records.
  
  
• 5). Mail or deliver your application, photo identification, check and a self-addressed stamped envelope to:

  Your marriage record will arrive two to four weeks after your application is processed if you apply by mail. If you apply in person, you will receive your marriage record the same day. Those who present two alternative forms of identification rather than a photo ID cannot obtain same day service when applying in person.

• 1). Visit the Vitalchek website to order a marriage record online (vitalchek.com). The State of Maryland authorizes Vitalchek to process online orders with a credit card for the purpose of obtaining marriage records.
  
  
• 2). Provide the information requested, including the information listed on the marriage certificate as well as your personal information, contact information and mailing information.
  
  
• 3). Choose a delivery method based on how quickly you want to receive your marriage record. The fee is $12 for a Maryland marriage record. Vitalchek charges an additional fee to process your credit card and your method of shipment is an additional fee added to your total order.
  
  
• 4). Provide Vitalchek with your credit card information and submit your order.
