- 1). Call your local department of motor vehicles and ask to have the disabled parking permit application mailed to your home. Some state offices allow you to retrieve and print out the forms online, so ask if this is an option.
- 2). Fill out the form in black or blue ink. Answer all questions giving as much information as possible. Make sure you include your driver's license number and handicap plate number if you've had one in the past.
- 3). Check the box designating you want a temporary parking placard. See if the application form lists a fee; for example, California charges $6 as of August 2011 for temporary handicap placards.
- 4). Complete the section regarding vehicle information for the car you drive. List the vehicle identification number that is found on your vehicle registration or on the metal plate attached to the inside of the vehicle's driver's side door. Fill in the boxes for make, model and license plate number.
- 5). Make an appointment to see your doctor. Every state requires information from your doctor specifying that you do have a physical impairment limiting your mobility. Make sure your doctor signs the application in the designated area on the form. Without a doctor's signature, your handicap permit is unacceptable.
- 6). Read the important information at the bottom of the form that explain penalties you face if you commit fraudulent acts while applying for or using the handicap plate or placard. Sign and date the form.
- 7). Read over the application form to make sure you've filled in all of the boxes. Correct any errors. Take note of the Department of Motor Vehicle's mailing address listed at the top or bottom of the application.
- 8). Photocopy your application and keep this copy for your records. Insert the application and payment into an envelope. Write the Department of Motor Vehicle's mailing address on the front of the envelope. Add a postage stamp and mail the application. If you're in a hurry, deliver the application and payment in person.
