Place an official acknowledgments page after the preface. Write: "The author(s)/editor(s) and publishers wish to thank the following for permission to reproduce copyright material." - 2). List out the permissions as given by the copyright holder. If the copyright holder did not provide the form he wishes you to use, use Modern Language Association, American Psychological Association, or another style manual as your guide.
- 3). Conclude the acknowledgments page with "Every effort has been made to trace rights holders, but if any have been inadvertently overlooked, the publishers would be pleased to make the necessary arrangements at the first opportunity." This statement should protect you if you accidentally failed to gain permissions on a copyright.
- 4). For illustrations, type the credit line at the bottom of the illustration. Give credit even if the copyright has expired to best protect yourself.
