How to File a Petition With the Ohio Tax Department

    • 1). Download the "Petition for Reassessment" form from the Ohio Department of Taxation website (see Resources).

    • 2). Fill out the "Petition for Reassessment" form. Leave no lines blank. Each field on the form must be filled in for a proper reassessment.

    • 3). Check the box in section "C" if you do not want the "Corrected Assessment" procedure. This procedure does not include a "Final Determination" by the Ohio Department of Taxation, which speeds up the appeals process.

    • 4). Send the form to the proper tax division within 60 days of the mail date of the assessment. The addresses of the different divisions are located on the form. If the Ohio Department of Taxation does not receive the petition within 60 days of the mail date, they will not review the assessment.
