- 1). Gather documents to prove citizenship, identity and residency. A U.S. passport, certificate of naturalization or citizenship, or Department of State consular report of birth abroad are all acceptable forms of proof of citizenship. A passport, state-issued driver's license, military or government issued ID, or tribal ID can be used as proof of identity. Residency can be proved with a utility bill, property tax bill, mortgage documents, homeowner's insurance policy, property deed, or concealed weapons permit.
- 2). Go to the state department of licensing or motor vehicles. Complete an application for an EID at the state department of licensing or motor vehicle office. You must enter your full name, date of birth, Social Security number and address on the application, and answer any additional questions.
- 3). Submit proof of citizenship, identity and residency with the completed application plus pay the fee for the EID. Fee varies per state.
- 4). Stand still for a photo at the licensing or motor vehicle office. The representative will take a photo to include with your EID. You must remove anything that covers your face or head like a hat or sunglasses.
- 5). Receive a temporary EID before you leave the office, and look for your permanent EID in the mail within two to three weeks.
