- 2). Provide the job applicant with the disclosure form for completion and authorization signature to obtain criminal records. The disclosure form has to be signed by both the employer and applicant. You have to extend an employment offer subject to a clean criminal record check before ordering the report. Submit the form to the Criminal Record Bureau.
